Why do safety investigation activities need to be implemented systematically?

Safety investigation is an important activity within the Safety Management System (SMS). At many units today, however, this activity still lacks a systematic character and has not yet fully realized its value in safety assurance.

Guidance on operating the Safety Management System SMS for investigators
Systematic safety investigation helps identify factors related to an incident and increases the value of investigation results in safety assurance.

In implementing and operating a Safety Management System (SMS) in practice, organizations often encounter a number of difficulties:
  • The concepts and approaches within the SMS are not yet clearly understood
  • Investigation procedures are not fully understood or are applied inconsistently
  • Lack of Tools and Methods to Support Analysis
  • Investigation reports remain descriptive and do not yet fully reflect the nature of the problem
These limitations can prevent investigation findings from delivering their full value in safety assurance. Equipping investigators with a common foundation in methods, processes, and tools is therefore the basis for investigations to be carried out in a more systematic way.

Program objectives

The Guidance on Operating a Safety Management System (SMS) for Investigators program equips participants with the foundation needed to carry out investigation activities within the SMS.
The program focuses on helping learners:
  • Understand the basic concepts of safety investigation in SMS
  • Understand the Investigation Process and Tools
  • Identify risks and hazards during the investigation process
  • Be able to prepare an appropriate investigation report.

Training content

The program comprises 7 core topics, connecting knowledge of the Safety Management System (SMS) with methods and tools for investigation:
  • Investigation model
  • Overview of the SMS
  • Investigation processes and tools
  • Risks and hazards
  • Reason model
  • Human Performance
  • Just Culture
Through these topics, participants gradually build the knowledge foundation needed to understand and carry out investigation within the SMS.

Guidance on operating the Safety Management System SMS for investigators
The program provides the knowledge foundation, processes, and tools investigators need to approach safety investigation systematically within the SMS.

Delivery approach

The program is designed to combine foundational theory, practice, and case study/scenario simulation. Trainees take part directly in exercises and situations in order to understand more clearly how to apply investigation processes, methods, and tools in practice. This approach connects what has been learned with the requirements that safety investigators may encounter in their work.

Value gained after the program

After completing the program, participants can:
  • Understand the investigation approach within the Safety Management System (SMS)
  • Understand investigation procedures and tools
  • Identify and analyze the factors related to the incident
  • Be able to prepare an investigation report that meets practical requirements.
The objective of the program is not only to provide knowledge of SMS and safety investigation, but also to help participants understand how to apply this knowledge, these methods, and these tools in actual work.

Which units is the program suitable for?

The program guiding operation of the Safety Management System (SMS) is suitable for:
  • Units that are implementing or completing a Safety Management System (SMS)
  • Safety investigators need to strengthen their knowledge and methods
  • Organizations that need to standardize investigation activities.

The program can be customized to actual operations

Each organization has its own operating model, people, and challenges, so an effective training program cannot be designed to a single common template.
Hung Viet customizes content, duration, and the method of delivery based on operational reality and the characteristics of each group of learners, so that knowledge does not stop in the classroom but can be turned into capability and applied at work.
